Many students need to take medication while at school. Please submit a Request to Administer Medication Form if your child needs to take medication while at school.

All prescription medication must have a signed Medication Form and be in the correctly labeled prescription bottle. The medication can only be given according to the directions on the label.

Non-prescription medication (Over the Counter) must also have a signed Medication Form and be in the original labeled bottle.

Action plans are the first step to understanding, and being prepared for, complex student health needs by documenting known health conditions and providing treatment guidelines.
 
The goal of an action plan is to provide school nurses and administrators with the necessary information to treat a student with certain health conditions.

Senate Bill (SB) 1506, 88th Texas Legislature, 2023, amends Texas Education Code (TEC) §38.032 requiring TEA to adopt and post a form on the agency’s website to be used in submitting a seizure management and treatment plan to a student’s school that includes a specific list of information and the signatures of the student’s parent or guardian and the physician responsible for the seizure treatment.   
 
This change in law only applies to a seizure management and treatment plan submitted to a school district or open-enrollment charter school on or after January 1, 2024.

If your child has a severe allergy and / or will have an Epinephrine pen at school, please provide the completed forms to your campus nurse. Please include any medical documentation from your physician.
